Privacy Policy
How we collect, use, disclose, safeguard, and provide access to personal information and personal health information.
Last updated: August 27, 2026
Parkside Drive Dental is committed to protecting your privacy. This Privacy Policy explains how our dental clinic collects, uses, discloses, stores, and safeguards personal information and personal health information when you visit our website, contact the clinic, complete forms, or receive dental care from us.
Our privacy practices are guided by Ontario's Personal Health Information Protection Act, 2004 (PHIPA), Canada's Personal Information Protection and Electronic Documents Act (PIPEDA), and other applicable privacy laws.
Information We Collect
Depending on how you interact with us, we may collect contact details such as your name, phone number, email address, mailing address, appointment preferences, messages you send to us, and information submitted through website forms.
When you become a patient, we may collect personal health information needed to provide dental care, such as dental and medical history, treatment notes, diagnostic images, insurance or payment information, referral information, consent records, and communications about your care.
Our website may also collect limited technical information, such as IP address, browser type, device information, pages visited, referring pages, and general usage data through standard server logs, cookies, analytics, and security tools.
How We Use Information
We use personal information and personal health information to respond to inquiries, schedule and confirm appointments, provide dental assessment and treatment, maintain patient records, communicate about care, process billing and insurance claims, meet legal and regulatory duties, improve our website and services, and protect the security of our systems.
We do not sell patient information. We do not use personal health information for unrelated marketing without consent where consent is required by law.
Consent and Your Choices
By providing information to the clinic, using our website, or receiving care from us, you consent to the collection, use, and disclosure of information as described in this policy and as otherwise permitted or required by law. You may withdraw consent for certain uses or disclosures, subject to legal, professional, and clinical record-keeping requirements.
Disclosure of Information
We may share information with dentists, hygienists, dental specialists, laboratories, pharmacies, insurers, payment processors, technology providers, professional advisors, regulators, or other parties when needed for care, clinic operations, billing, legal compliance, or patient-authorized purposes.
Where service providers support our website, scheduling, communications, records, analytics, or security, they are expected to handle information only for authorized purposes and with appropriate safeguards.
Cross-Border Processing and Storage
Some of the service providers that support our website, communications, scheduling, analytics, security, payment processing, or clinic operations may process or store personal information or personal health information outside Ontario or Canada, including in the United States. When information is handled outside Canada, it may be subject to the laws of that jurisdiction, including lawful access by courts, law enforcement, or government authorities.
We use reasonable contractual, administrative, and technical safeguards intended to protect information handled by service providers and to limit their use of the information to authorized purposes.
Safeguards
We use administrative, physical, and technical safeguards intended to protect information against unauthorized access, use, disclosure, loss, or theft. These safeguards may include access controls, staff confidentiality expectations, secure systems, and reasonable record-handling practices.
Data Retention
Clinical records and personal health information are retained in accordance with Ontario law and professional requirements, including Royal College of Dental Surgeons of Ontario guidance. Dental records are generally retained for at least 10 years from the date of the last entry in the record; for minors, records are generally retained for at least 10 years after the patient reaches the age of majority.
Website form submissions, appointment requests, and general inquiries are retained only for as long as reasonably needed to respond, manage scheduling or follow-up, maintain appropriate business records, and meet legal or regulatory obligations. Information submitted through online forms may be transferred into our clinic communication, scheduling, or record systems when needed for those purposes.
Server logs, analytics data, security records, and similar technical information are retained for limited periods needed for website operations, security, troubleshooting, analytics, and legal compliance, after which they are deleted, de-identified, aggregated, or overwritten according to the practices of the clinic and its service providers.
Data Deletion and Disposal
When personal information or personal health information is no longer needed for the purposes described in this policy, and no law, professional duty, or clinic record-keeping requirement requires continued retention, we take reasonable steps to securely delete, destroy, de-identify, or anonymize it. Paper records, electronic records, backups, and service-provider records may be handled on different schedules, but disposal is carried out using safeguards appropriate to the sensitivity of the information.
Access, Correction, and Deletion Requests
You may request access to, or correction of, your personal information or personal health information in accordance with applicable law. You may also request deletion of information where deletion is legally and professionally permitted. We may need to verify your identity before responding and may refuse or limit access, correction, or deletion where the law permits or requires us to do so, including where records must be retained for legal, professional, clinical, or regulatory reasons.
